Strategies for supporting clinicians providing HIV care and treatment in resource limited countries

Introduction: The International Center for AIDS Care and Treatment Programs (ICAP) at Columbia University´s Mailman School of Public Health supports multinational programs to expand access to HIV/AIDS care and treatment in resource-limited settings. These programs include Columbia´s MTCT-Plus Initiative, University Technical Assistance Program (UTAP), and the Multicountry Columbia Antiretroviral Program (MCAP); ICAP currently supports HIV/AIDS care and treatment for adults and children in 13 countries.

Methods: We describe the levels and types of clinical support provided to ICAP sites and programs in order to illustrate the nature of technical assistance and clinical consultation required by nascent HIV/AIDS treatment programs.

Results: Clinical support included:
(a) The design of step-by-step treatment protocols and policies consistent with local, national, and WHO guidelines;
(b) The development of clinical support tools, manuals such as The Columbia Clinical Manual, training materials, pocket cards, and other job aids;
(c) Provision of regular technical updates to site and program staff;
(d) On-site assessment and programmatic consultation, and;
(e) Rapid access to specialist consultation via email.
Conclusions: Multi-level clinical and technical input is required to support high-quality clinical care at pilot HIV/AIDS care and treatment programs. As these sites develop, they can provide additional support to local and national programs.
