Changes in the pattern of PLWA volunteer activities in MTCT-Plus clinics after introducing PLWA guidelines

Background: PLWA peer group has important roles in multidisciplinary care team in giving psychosocial and adherence supports to MTCT-Plus patients and family. The Thai Red Cross AIDS Research Centre (TRCARC) has introduced PLWA guidelines into MTCT-Plus programs in 2005 to help define the pattern of activities given by PLWA volunteers and set up appropriate wages for those activities. We would like to study the impact this guidelines may have on PLWA activities and patient satisfaction in MTCT-Plus clinics.

Methods: PLWA volunteers working in MTCT-Plus clinics were asked to fill out questionnaires about changes in the pattern of activities, number of activities done to meet patient’s demand, and wages. MTCT-Plus patients were also randomly asked to fill out questionnaire about satisfaction on PLWA volunteer activities.

Results: Of 16 staff working in MTCT-Plus clinics, 50.0%, 62.5%, 37.5%, 25.0% thought that the guidelines help define the pattern of individual counseling, telephone counseling, group activity, and home visit respectively. The proportions of staff who felt that after having guidelines they could increase the number of activities done to meet patient’s demand were 56.3% for individual counseling, 68.8% for telephone counseling, 43.8% for group activity, and 25.0% for home visit. In addition, 3 out of 16 thought that they received higher wages for all activities.
Using a score of 0-3 (not satisfied, somewhat satisfied, very satisfied, most satisfied), 42 patients rated their satisfaction on PLWA volunteer activities in MTCT-Plus programs as 2.3 for individual counseling, 2.3 for telephone counseling, 2.0 for group activity, and 2.3 for home visit.

Conclusions: After introducing PLWA guidelines, PLWA volunteers worked more happily and more effectively in MTCT-Plus programs and patient satisfaction was very high in all activities carried out by PLWA volunteers.
